Eligibility Requirements 
 

1. Graduating Male or Female of a Northern Illinois High School

2. Grade Point Average of 2.5 or higher

3. Plan to attend a) Junior College,  or  b) Division III School

4. Plan to Play Intercollegiate Basketball, or Major in Sports Management, or Major In Education

Materials Required 

1. Completed Application Form

2. Copy of 6th semester transcript with Cumulative GPA

3. Written statement explaining future plans and goals

4. Signed letter of recommendation from a school staff member

5. Name of at least one other staff member who can be contacted

Award Amount 

Amount of award will be in the $500-$1500 range and

depends on the number of recipients and funds available.
